Athletic Clearance Process

The Athletic Clearance Process is the first step to becoming a student-athlete at Los Amigos High School. The process must be completed every school year. Students will not be allowed to participate in practice or games until all clearance items are completed.
  1. Complete all forms on
    • Register for a new account or Login (if you already have one)
    • Click the Start Clearances Here button
    • Select the school year, Los Amigos (CIF-SS), and your sport
    • Step 1: Complete the student-athlete information
      • If you do not have a family doctor, choose N/A under Physician
      • At Physical Uploads step, just click Save
    • Step 2: Complete Medical History and click Save
    • Step 3: Complete Parent/Guardian info and click Submit
    • Step 4: Read each text box containing information
      • Parents: "sign" by typing your name in each Signature box
      • Students: "sign" further down by typing your name in each Signature box
    • When you see the green text "You have successfully completed" you are done
    • Optional: You may print out the confirmation to give to your coach
    • Multiple languages are available from the drop-down at the top of each page

  2. Complete a Physical Exam
    • Physicals are valid for one calendar year from the date of the exam
    • List of Sports Physical Locations near Fountain Valley / Santa Ana / Garden Grove
    • Print out the Physical Form and bring to your exam
    • Forms are also available in the Front Office of LAHS
    • Turn in the signed completed Physical form to your Coach, Front Office, or AD
  3. Complete an Acknowledgement of Risk Form (Transfer or Incoming Freshman only)
    • Form is valid for four years (no need to re-sign every year)
    • Parent and student must sign in the presence of a school official
    • Forms are available in the Front Office of LAHS

  4. Maintain a 2.0 GPA in all classes attempted during the last grading period

  • Each athlete must print and read one of the Parent Letters (English / Spanish / Vietnamese)
  • Please have your parents read and sign the letter where indicated
  • Bring the signed Parent Letter to the first day of practice for your sport
