The Athletic Clearance Process is the first step to becoming a student-athlete at Los Amigos High School. The process must be completed every school year. Students will not be allowed to participate in practice or games until all clearance items are completed.
REQUIREMENTS FOR ATHLETIC CLEARANCE
- Complete all athletic clearance forms on Homecampus.com
- Create a new account or Login (if you already have one)
- Click the Start Clearances Here button
- Select the school year, Los Amigos (CIF-SS), and your sport or sports if you are a multi sport athlete
- Step 1: Complete the student-athlete information
- If you do not have a family doctor, physician's phone number or preferred hospital choose N/A
- Step 2: Complete Parent/Guardian info and click Save and Continue
- Step 3: Complete Medical History and click Save and Continue
- Step 4: Read each text box containing information
- Parents: "sign" by typing your name in each Parent Signature box
- Students: "sign" by typing your name in each Student Signature box
- Step 5: At Files Step you do not need to download or upload, just click save and continue
- Step 6: Print Confirmation Message page, have parent sign, and hand to Athletic Director
- Complete a Physical Exam
- Physicals are valid for one calendar year from the date of the exam
- List of Sports Physical Locations near Fountain Valley / Santa Ana / Garden Grove. Walgreens, CVS, and some urgent care facilities are now offering sports physicals starting at $20.
- Print out the Physical Form and bring to your exam
- Forms are also available in the Front Office of LAHS
- Turn in the signed completed Physical form to your Coach, Front Office, or AD
- Complete an Acknowledgement of Risk Form
- Form is valid for four years (no need to re-sign every year)
- Parent and student must sign in the presence of a school official
- Forms are available in the Front Office of LAHS
- Maintain a 2.0 GPA in all classes attempted during the last grading period
